Please bring the following items with you to your appointment:
1. Your eyewear
2. A list of any medications you are currently taking, including eye drops
3. Your insurance card(s) or form(s)
You will also be asked to provide medical history information that your physician will use to determine your course of treatment. For your convenience, you may click here to print the medical history form and fill it out before you arrive. A comprehensive eye examination may take several hours and might include dilating (enlarging) the pupils with drops. The drops may cause temporary blurring of vision (especially for reading) and sensitivity to bright light. Therefore, we recommend that you make arrangements for transportation after your appointment.
Referrals If you are referred to our office by another medical doctor or optometrist for a consultation, you can be assured your visit will be followed up with a written report or personal phone call to your doctor. The consulting physician will be kept up to date on your medical or surgical treatment, and you will return back to them at the appropriate time.
